AHCCCS describes its enforcement response as targeting up to $2.5 billion in Medicaid fraud.



The Attorney General’s May 14, 2026 update described the broader scheme as involving losses reaching into the billions. Reporting tied to that update places the estimate at approximately $2.8 billion.

How Much Money Was Involved?

AHCCCS reported opening only nine new behavioral-health fraud investigations in 2019.



The small number later became an important comparison point. By 2023, new investigations had risen to approximately 1,400. That increase reflected both the scheme’s growth and the state’s delayed identification of suspicious providers and billing.



The 2019 figure is a count of investigations, not a count of providers, convictions or confirmed fraudulent claims.

AZCIR and ProPublica identified at least 40 Indigenous residents of Phoenix-area sober-living homes and treatment facilities who died between 2022 and 2024.



Their reporting connected the deaths to the broader system in which fraudulent providers recruited vulnerable people, billed for questionable or nonexistent care and exposed patients to unsafe conditions.



AHCCCS has not published its own comprehensive count of deaths connected to the fraud scheme. The 40-death figure is therefore an investigative reporting count rather than an official agency total.
