Fraud, Abuse, Improper Payments, and Audit Findings

The <a href="https://www.justice.gov/usao-mn/pr/defendants-charged-first-wave-housing-stabilization-fraud-cases">U.S. Attorney’s Office for Minnesota announced the first Housing Stabilization Services charges in September 2025</a>.



The charging documents alleged that providers obtained and misappropriated Medicaid funds intended to help people find and maintain housing. Prosecutors described low provider-entry barriers and limited reimbursement documentation as conditions that increased exposure to false claims.



The program began in 2022. Minnesota’s Department of Human Services later announced that it would terminate the benefit, and the program ended at 11:59 p.m. on October 31, 2025.

DHS also said it was coordinating with counties, Tribes, urban Indian organizations, and managed-care organizations to connect affected participants with other resources.



The program closure is a documented administrative action. The criminal allegations remain tied to individual charging documents and court proceedings.

What Did Housing Stabilization Services Reveal?

The Feeding Our Future case involved federally funded child-nutrition money administered through Minnesota’s education department.



According to the <a href="https://www.auditor.leg.state.mn.us/ped/2024/feeding-our-future.htm">Minnesota Office of the Legislative Auditor’s special review</a>, the Department of Education’s oversight created opportunities for fraud. The review examined warning signs, oversight decisions, complaint handling, and the department’s response to concerns involving participating organizations and meal sites.



Federal prosecutors charged participants with submitting false records and seeking reimbursement for meals that were not served. By March 2026, 79 people had been indicted and 63 had been convicted. One defendant received a 12-year sentence and was ordered to pay more than $47 million in restitution.

A Minnesota-specific review would need to connect loans to business registrations, payroll records, addresses, forgiveness decisions, civil settlements, criminal cases, and recovered funds.

What Did PPP and EIDL Records Show?

Advance premium tax credits depend on projected annual income, household size, enrollment, and access to other qualifying coverage.
