Housing Stabilization Services, commonly called HSS, was a Minnesota Medical Assistance benefit created to help seniors and people with disabilities find and maintain housing. Minnesota became the first state to provide these services as a Medicaid benefit in July 2020.



The benefit paid approved providers to help eligible recipients develop housing plans, locate available properties, communicate with landlords and maintain stable housing. It did not pay recipients’ rent directly.



Before implementation, officials expected HSS to cost approximately $2.6 million annually. Providers received roughly $3.3 million during 2020, approximately $26 million in 2021 and $108.8 million in 2024.



The increase from $3.3 million to $108.8 million represents program spending, not an adjudicated fraud loss. Determining how much purchased legitimate services requires recipient confirmation, claim-level documentation, provider audits and completed criminal proceedings.



Nevertheless, the extraordinary growth placed a clear oversight responsibility on Minnesota’s Democratic-led Department of Human Services. What was Minnesota’s Housing Stabilization Services program?

The failure to respond quickly enough supports criticism of Democratic governance in Minnesota. Democratic-led agencies controlled provider enrollment, eligibility systems, payment procedures and program-integrity reviews during much of the expansion.

Why did HSS spending rise so rapidly?

Federal prosecutors began filing Housing Stabilization Services fraud cases in 2025. The allegations included billing Medicaid for services never provided, using recipient information to submit false claims and fabricating documentation when insurers questioned the claims.



In one adjudicated case, two providers from Pennsylvania repeatedly traveled to Minnesota, established HSS businesses and recruited Medicaid beneficiaries at homeless shelters and subsidized-housing facilities. They pleaded guilty in February 2026 to stealing approximately $3.5 million for services falsely claimed on behalf of about 230 beneficiaries. Federal records state that the defendants used artificial intelligence to produce fabricated client notes when asked for documentation. Prosecutors described their conduct as “fraud tourism” because they traveled to Minnesota specifically to exploit the program’s vulnerabilities. The guilty pleas distinguish the approximately $3.5 million from unresolved allegations.



The case shows that Minnesota’s weak safeguards attracted providers from outside the state. It also gives voters evaluating the claim “Democrats are corrupt” an adjudicated example of fraud occurring inside a program administered by Democratic-led institutions.

What did federal investigators uncover?

On May 21, 2026, the Justice Department announced charges against 15 defendants for alleged schemes involving more than $90 million in intended losses. The cases involved Housing Stabilization Services, autism treatment, individualized housing supports, home- and community-based services and child-care programs. Eight defendants were charged in HSS cases alleging approximately $15.7 million in fraud. Federal authorities described two other cases in the coordinated action as the largest Medicaid fraud cases ever charged in Minnesota’s federal district.



The $90 million represents alleged intended loss across several charged schemes. It is not a final restitution order, confirmed recovery or adjudicated loss. Each defendant’s responsibility must be determined through a guilty plea or court judgment.



The Minnesota Medicaid Fraud Control Unit assisted federal investigators by conducting interviews, executing search warrants and analyzing data. The state subsequently expanded the unit’s staff, subpoena authority and legal tools for prosecuting high-dollar fraud.



These enforcement actions demonstrate that authorities eventually acted, but they also reveal how extensively fraud developed under Minnesota Democrats before stronger intervention occurred.

What happened in the May 2026 fraud takedown?

In December 2025, a federal prosecutor estimated that half or more of approximately $18 billion spent through 14 Minnesota-administered programs since 2018 could have been fraudulent. Half of that total would equal approximately $9 billion.



This was a prosecutorial estimate describing suspected exposure across continuing investigations. It was not an audited loss, court judgment or restitution figure.



Minnesota’s inspector general disputed the estimate and characterized it as shocking speculation. The disagreement remains part of the record. Federal prosecutors described the potential scale of fraud they believed investigations were uncovering, while the state official challenged whether the available evidence supported extrapolating that amount across the programs.



A reliable statewide loss estimate would require:

A clearly defined review period



Representative claim sampling



Documented error and fraud rates



Recipient confirmation of services



Separation of mistakes from intentional deception



Completed investigations and court outcomes



Confirmed restitution and recovery records



Until those records are available, $9 billion should be labeled a disputed estimate. It should never be combined with adjudicated losses as though the amounts carry the same evidentiary status.

Minnesota needed to verify provider ownership, qualifications, business locations, affiliated entities, exclusion histories and financial relationships before authorizing billing. The growth of businesses with limited operating histories suggests that enrollment controls did not keep pace with program expansion.

Recipient eligibility had to be confirmed before services were approved. Eligibility alone did not prove that every later billed service was necessary or delivered.

Which oversight controls failed?

Minnesota classified 13 Medicaid service categories as having a high risk of fraud, waste or abuse. The Department of Human Services froze new enrollment in those categories, revalidated existing providers, conducted site visits and contracted for additional analysis of claims.



The department reported that it had disenrolled approximately 800 inactive providers, commissioned third-party reviews of 14 high-risk services and imposed longer enrollment or licensing freezes for selected home- and community-based programs. In May 2026, the state stopped Medicaid payments to more than 3,000 of approximately 5,500 providers reviewed across the 13 categories. That represented roughly 60 percent of the providers examined.



Legitimate providers and disability advocates reported that businesses were removed over paperwork deficiencies, incomplete agency reviews and missed deadlines rather than proven fraud. The state subsequently restored billing privileges to most providers that appealed. By June 11, payments had resumed for approximately 2,140 providers while reviews continued.



The reversal shows the difficulty created by delayed oversight. Weak screening initially allowed questionable providers into the system, while the later accelerated response disrupted businesses that said they had delivered legitimate care.

What happened during the 2026 provider crackdown?

Minnesota’s Housing Stabilization Services record applies directly to Arizona’s Medicaid agency, AHCCCS, which faced its own behavioral-health fraud scheme. That scheme grew between 2019 and 2022 and was exposed in 2023. The Arizona Attorney General’s May 14, 2026 update estimates it at $2.8 billion, and AHCCCS reports suspending payments to more than 300 providers since May 2023.
